Logic Model And Evaluation Planning Grant
Project Overview
The Logic Model & Evaluation Planning (LME) Project, facilitated in partnership with You Thrive Training & Consulting, LLC, is the second phase of the Community Planning Framework Initiative which was launched in Fiscal Year 2025.
Building on insights from conducting Community Readiness Assessments (CRA), this phase supports participating agencies in translating their CRA findings into clear, actionable prevention plans. Through guided facilitation and structured planning, agencies will identify and prioritize key focus areas based on CRA findings; and, clarify goals and intended outcomes for prevention efforts. Rather than moving directly into implementation, the LME process focuses on helping agencies define priorities, align strategies, and establish measurable outcomes that reflect their community’s needs.
Logic models and evaluation planning are pivotal as they turn ideas into work that can be tracked, understood, and improved. A logic model offers a clear, visual way to show how an agency's resources, planned activities, and intended outcomes connect; as well as creating a shared understanding of the work and why it matters across staff, leadership, and partners.
Evaluation planning builds on that foundation by defining how progress will actually be measured, helping agencies confirm they're reaching their intended results, identify what isn’t successful, and speak to funders and boards with clear evidence of their impact. This data-informed planning will advance problem gambling prevention services across Ohio, supporting agencies in best meeting community needs.
This process helps agencies move from insight to action, creating a strong foundation for prevention efforts that are intentional, data-informed, and sustainable.
By the end of the project,
agencies will:
- Develop a logic model grounded in their community’s readiness data and clearly aligned with identified priorities
- Create a structured evaluation plan that defines meaningful indicators, data sources, and processes for tracking progress and outcomes
- Strengthen their ability to connect data to decision-making
- Build confidence in communicating their strategy, including clearly explaining the rationale behind their goals and selected strategies to advocate for better protection in their communities
- Establish stronger collaboration across internal teams, leadership, and partners by creating a shared understanding and direction
- Leave with practical tools and planning documents that support implementation, reporting and stakeholder engagement
The LME Project allows for agencies to move from understanding their community to implementing a clear, strategic approach to prevention.
