Agency Service Consultation Application
Selected agencies will receive guidance from PGNO to develop, prioritize, and implement organization-wide strategies for embedding gambling-related services. Each agency begins by assessing and identifying organizational goals, prioritizing needs, and selecting an approach. This process may include targeted trainings and workforce development that guide the creation of tailored integration plans. The first phase of the project culminates in a year-end report event and outcome evaluation presentation, where agencies reflect on progress, share lessons learned, and build momentum for future phases.
Eligibility
- Must be an agency or organization providing services in the state of Ohio.
- An agency or organization must be interested in more fully integrating problem gambling treatment or prevention services into internal systems such as policy, procedures, workforce development, marketing, etc.
- Full participation from 4-6 Team Members that hold a lower-level management role or higher.
- Willingness to participate in the consultation process and be open and honest in sharing information, providing feedback, and collaborating to identify needs, challenges, and opportunities for improvement.
Project Timeline
October 10, 2025 – June 26, 2026

Grant Funds
Up to four agencies will receive a $15,000 grant to be used to cover staff time at mandatory meetings and to aid the completion and implementation of agreed-upon Allocation Deliverables submitted by the applicant.
PGNO may offer additional funding in future years to support further development of service consultation goals as part of a multi-phase project.
Application Requirements:
1. Organizational Information
- Name, address, website, and mission of your organization
- Counties in the state of Ohio that you serve
- Summarize the history and evolution of your organization and its commitment to public health and community issues, particularly problem gambling
- List the programs and initiatives your organization currently runs
- Highlight key achievements and successes of your organization in the area of community health, prevention, and treatment
- Provide examples of organizational buy-in for this project
- Discuss how this grant aligns with current efforts and your organization’s overall mission
2. Team Member Information
- List the four to six team members who will be involved in the Agency Service Consultation process. One team member will be the team lead.
3. Demonstration of Interest & Need
- What do you perceive are the key challenges your community may face regarding problem gambling?
- Why does your agency view participation in the consultation process as beneficial? Please describe how engaging in consultation could support your organization’s goals, strengthen internal systems (such as policy, workforce development, or service delivery), and enhance your capacity to address problem gambling prevention and treatment.
- What opportunities does your agency see for improving or strengthening problem gambling services within your organization? Please describe 2-4 areas where consultation could support growth in treatment, prevention, or organizational systems.
4. Sustainability Concepts
- What leadership or organizational support will be in place to ensure continued engagement in problem gambling services after the grant ends? Discuss how your organization plans to maintain focus on addressing problem gambling, including any strategies or resources that will be used to support ongoing efforts.
5. Budget Template & Narrative
