Agency Service Consultation Phase 1 Application

The Agency Service Consultation uses a consultation model to assist prevention and treatment agencies across Ohio in integrating gambling prevention and treatment strategies into their organizational culture and service delivery. The goal is a strengthened approach to addressing gambling-related harms across prevention, care, and support services statewide.


PGNO works directly with agency leadership on systemic change to embed gambling disorder prevention and treatment into agency systems and culture. We focus on all aspects of workforce development and service delivery: from intake and assessment to referral pathways, clinical and prevention services, marketing and communications, and internal policy. 


This application will help us understand your agency’s scope of work, history with problem gambling services, and fit with the Agency Service Consultation Program. The information captured demonstrates where your agency stands today across prevention, treatment, partnerships, data practices, and leadership culture which will be used to shape Phase I. After the site visit and initial consultation meeting, your agency will work with PGNO to identify priorities and set specific goals for the consultation ahead. The first phase of the project culminates in a year-end report and outcomes presentation, where agencies reflect on progress, share lessons learned, and build momentum for future phases.

Eligibility

  • Must be an agency or organization providing services in the state of Ohio.
  • An agency or organization must be interested in more fully integrating problem gambling treatment or prevention services into internal systems such as policy, procedures, workforce development, marketing, etc. 
  • Full participation from 4-6 Team Members that hold a lower-level management role or higher.
  • Willingness to participate in the consultation process and be open and honest in sharing information, providing feedback, and collaborating to identify needs, challenges, and opportunities for improvement.

Project Timeline

October 21, 2026 – June 30, 2027

Grant Funds

Up to four agencies will receive a $15,000 grant to support staff time for required meetings and participation throughout Phase I.


PGNO may offer additional funding in future years to support agencies continuing into later phases of the project.

Application Requirements:

The full application takes approximately 45 minutes to complete. The purpose is to help PGNO understand where your agency is today and how you might fit with the Agency Service Consultation process.


Before you begin: a few things are worth having ready so your team can complete the application in one sitting.


  • Your primary point of contact's information, including email address and telephone number. 
  • Basic details about your agency, including size, funding sources, and history with problem gambling-specific services.


The application itself is organized in four parts:


  • 1. Agency Information

    • Agency name, primary point of contact, counties served, primary populations served, geographic classification, agency makeup,and funding sources.
    • Your agency's history with problem gambling-specific services. 
    • Your agency's interest in Agency Service Consultation, any prior staff or leadership exposure to problem gambling training.
    • Your programmatic map is requested here.

  • 2. Agency Self-Assessment

    A rating-based reflection on where your agency currently stands across six areas:

    • Current capacity 
    • Prevention-specific practices
    • Treatment-specific practices
    • Partnerships and community engagement
    • Data and quality improvement
    • Leadership and culture. 

    Ratings run from 1 (not in place) to 5 (strong), with space to add context after each set of items. This scale is a shared reference point for our understanding or programs currently in place. 


  • 3. SWOT Analysis

    • Identify your agency's top 2–3 strengths, weaknesses, opportunities, and threats related to problem gambling services.

  • 4. Budget Template & Narrative

    • In addition to the application above, provide a budget and narrative showing how the $15,000 grant funds would be allocated (e.g., staff time, travel costs, materials). For guidance, please review this sample budget template.

Please submit your completed application, programmatic map, and budget template together by Monday, September 21, 2026. Applications submitted after this date may not be considered.


Applicants will be notified of their selection status by Monday, October 5, 2026.
